General Leave

- Employees who have been employed for 90 days or more have the ability to request a General Leave of Absence.

- Employee provides written or oral notice of the service to HR (at least 30 days if possible)
- Employee submits Request for General Leave of Absence form with HR signature to supervisor and Department Head/Director for approval

- General leaves are not used to extend vacations
- General leaves may be granted for a period of up to 30 days and extended under special circumstances by the Department Head/Director and HR Director
- Accrued PTO may be used until the balance is used and the rest of leave can be unpaid
- Employees do not accrue PTO during general leaves 
- Employees are responsible for their portion of health and insurance premiums for continued coverage
- Efforts to return the employee to the same position are made after leave ends, however, reinstatement to original position cannot be guaranteed 

- You are responsible for reporting for work at the end of an approved general leave
- You will voluntarily resign employment if you:
  - Fail to report to work on the return to work date
  - Accept other employment during the leave
  - Cannot obtain a position within 30 days of return if their original position is no longer available

* Comply with all terms and conditions of the granted leave
* Arrange to pay benefit cost or premiums (if applicable)
* Report changes to the leave to HR as soon as possible

- Employee notifies HR of intent to return to work at end of an approved general leave
- If employee fails to do so, they will be considered voluntary termination

- Direct employees to consult with HR on the terms and conditions of general leave
- Communicate with the HR Leave Representative during leave
- Follow any directive from HR regarding the employee’s return to work

- Employee submits request for leave in writing to supervisor
- Employee provides any requested supporting documentation
- Supervisor reviews and approves leave of absence after consulting with HR
- Supervisor initiates a Personal Change Notice (PCN) or equivalent and forward it with supporting documentation to HR
- HR processes a PCN or equivalent and file in employee’s personnel file
- Employee comply with all terms and conditions of the granted leave
- HR processes a PCN or equivalent when the employee returns from leave
- HR initiates termination procedures if employee fails to return from leave
